Active Memberships

Initial term of your agreement is 12 months and non-refundable

Effective on the first Membership Charge Date

You may not transfer any account credit or memberships to any other patients

All treatment and account balances not utilised during a Monthly Period may be carried over to a subsequent Monthly Period for a period of up to 12 months if account is current and in good standing but may not be carried over beyond thirty (30) days of the cancellation of the Membership Program and no refund(s) will be issued.
